サーバーがこの値を割り当て、クライアントが指定した値を無視する。ただし、サーバーが更新/削除時にバージョンの整合性を強制する場合を除く。

recorder要素から参照される場合には、そのJP_Practitionerリソースの実体。JP_Practitionerリソースにおける必要最小限の要素だけが含まれればよい。

【電子カルテ情報共有サービス（5情報送信）では必須】本情報を作成発行した医療機関の識別番号を記述するために使用する拡張「eCS_InstitutionNumber」。 本情報は、ServiceRequestの要素として記述することも可能であるが、その場合もこの拡張で記述することとする。

電子カルテ情報サービスでは、この拡張による記述は必須。医療機関１０桁番号を示すsystem値は"http://jpfhir.jp/fhir/core/IdSystem/insurance-medical-institution-no"を使用する。

【電子カルテ情報共有サービス（5情報送信）では必須】本情報を作成発行した診療科または作成発行者の診療科情報を記述するために使用する拡張「eCS_Department」

電子カルテ情報サービスでは、この拡張による記述は必須。コード化する場合には、JAMI(SS-MIX2) 診療科コード表のsystem値"http://jami.jp/SS-MIX2/CodeSystem/ClinicalDepartment"を使用する。診療科を記述する場合には、そのコード化の有無に関わらずtext要素による記述は必須。

主傷病フラグのある傷病名であることを記述するための拡張「eCS_Department」。電子カルテ情報共有サービスでは主傷病には設定必須。

system値：http://terminology.hl7.org/CodeSystem/ex-diagnosistype、value値：principal　を設定する。

コード化記述が必須。clinicalStatusとの制約条件を参照のこと。疑い病名フラグとしても使用される。 疑い病名フラグがない病名には、confirmed を設定するが、一度登録された病名を取り消した場合やそれが入力エラーであったことを明示したい場合には、refuted（取り消された病名）、entered-in-error（誤って登録された病名）のいずれかを設定することもできる。 疑い病名フラグのある病名には、unconfirmed を必ず設定する。 なお、電子カルテ情報共有サービスでは、confirmed、unconfirmed以外の病名に対してはその情報は受信データとして保存されるが、利用されたり表示されたりすることはない。過去の登録病名情報に対して削除や取り消しなどの処理を行うために使われることもないため、refutedやentered-in-errorをつけて送信しても過去に送信した病名を取り消すことはできない。

この要素は、このリソースのデータを取り込んで蓄積していたシステムが、このリソースになんらかの変更があった可能性があった日時を取得し、このデータを再取り込みする必要性の判断をするために使われる。本要素に前回取り込んだ時点より後の日時が設定されている場合には、なんらかの変更があった可能性がある（変更がない場合もある）ものとして判断される。したがって、内容になんらかの変更があった場合、またはこのリソースのデータが初めて作成された場合には、その時点以降の日時（たとえば、このリソースのデータを作成した日時）を設定しなければならない。内容の変更がない場合でも、このリソースのデータが作り直された場合や単に複写された場合にその日時を設定しなおしてもよい。ただし、内容に変更がないのであれば、日時を変更しなくてもよい。また、この要素の変更とmeta.versionIdの変更とは、必ずしも連動しないことがある。

入院外来区分情報。encounter要素から参照されるJP_Encounterリソースの実体。JP_Encounterリソースにおける必要最小限の要素だけが含まれればよい。ここで埋め込まれるJP_Encounterリソースでは、Encounter.classにこの情報を記録したときの受診情報（入外区分など）を記述して使用する。電子カルテ情報サービスでは必須。

傷病名情報の記録者情報をコンパクトに格納したPractitionerリソース

recorder要素から参照される場合には、そのJP_Practitionerリソースの実体。JP_Practitionerリソースにおける必要最小限の要素だけが含まれればよい。

【電子カルテ情報共有サービス（5情報送信）では必須】本情報を作成発行した医療機関の識別番号を記述するために使用する拡張「eCS_InstitutionNumber」。 本情報は、ServiceRequestの要素として記述することも可能であるが、その場合もこの拡張で記述することとする。

電子カルテ情報サービスでは、この拡張による記述は必須。医療機関１０桁番号を示すsystem値は"http://jpfhir.jp/fhir/core/IdSystem/insurance-medical-institution-no"を使用する。

【電子カルテ情報共有サービス（5情報送信）では必須】本情報を作成発行した診療科または作成発行者の診療科情報を記述するために使用する拡張「eCS_Department」

電子カルテ情報サービスでは、この拡張による記述は必須。コード化する場合には、JAMI(SS-MIX2) 診療科コード表のsystem値"http://jami.jp/SS-MIX2/CodeSystem/ClinicalDepartment"を使用する。診療科を記述する場合には、そのコード化の有無に関わらずtext要素による記述は必須。

主傷病フラグのある傷病名であることを記述するための拡張「eCS_Department」。電子カルテ情報共有サービスでは主傷病には設定必須。

system値：http://terminology.hl7.org/CodeSystem/ex-diagnosistype、value値：principal　を設定する。

このリソース情報の識別ID。

リソース一意識別IDの仕様は、を参照のこと。

リソース一意識別IDのsystem値は"http://jpfhir.jp/fhir/core/IdSystem/resourceInstance-identifier"　を設定する。

「リソース一意識別ID」の文字列。URI形式を使う場合には、urn:ietf:rfc:3986に準拠すること。

臨床的状態。病名最終日（abatementDateTime)での状態（転帰）。コードでの記述は必須。ただし、verificationStatus要素が'entered-in-error'であれば、本要素は存在してはならない。それ以外では、必須。

コード表　http://terminology.hl7.org/CodeSystem/condition-clinical　から　active（存続）, remission(軽快、寛解), inactive（治癒以外での病名の終了）→CLINSでは使用しない, resolved (治癒) 、unknown（不明）のいずれかを選ぶ（軽快は状況に応じてremissionまたはresolvedを使用）。

コードだけでは記述できない情報がある場合にコードと併用してもよい。値が使用されない可能性はある

unconfirmed | confirmed | refuted | entered-in-error のいずれか（未確認、確認ずみ、否定、エラー）。コメントを参照のこと。コード化の場合のsystem=http://terminology.hl7.org/CodeSystem/condition-ver-status

コード化記述が必須。clinicalStatusとの制約条件を参照のこと。疑い病名フラグとしても使用される。 疑い病名フラグがない病名には、confirmed を設定するが、一度登録された病名を取り消した場合やそれが入力エラーであったことを明示したい場合には、refuted（取り消された病名）、entered-in-error（誤って登録された病名）のいずれかを設定することもできる。 疑い病名フラグのある病名には、unconfirmed を必ず設定する。 なお、電子カルテ情報共有サービスでは、confirmed、unconfirmed以外の病名に対してはその情報は受信データとして保存されるが、利用されたり表示されたりすることはない。過去の登録病名情報に対して削除や取り消しなどの処理を行うために使われることもないため、refutedやentered-in-errorをつけて送信しても過去に送信した病名を取り消すことはできない。

コードだけでは記述できない情報がある場合や、コード化できない場合には本要素で記述する。コードと併用してもよい

臨床的状態に割り当てられたカテゴリー。設定する場合には、problem-list-item （プロブレムリスト）| encounter-diagnosis （診察時点での診断名）のいずれかを設定する。電子カルテ情報共有サービスでは、コード'encounter-diagnosis'とすること。コードに対応するdisplay nameは"Encounter Diagnosis"。

"記述する場合は、コード表："http://jpfhir.jp/fhir/core/CodeSystem/JP_ConditionSeverity_CS"からMI：軽度、MO：中度、SE：重度、UK：不明のいずれかを設定する。"

傷病名のコードと名称。ICD10対応標準病名マスタの「病名管理番号（system値は"http://medis.or.jp/CodeSystem/master-disease-keyNumber"）を必ず使用する。なお、病名のコード化ができない場合には、病名管理番号と同じ桁数の全桁9の文字列を設定する。なお、病名管理番号以外のコード記述を併用しても構わない。(参考：MEDIS 病名交換コード：http://medis.or.jp/CodeSystem/master-disease-exCode、ICD10分類コード：http://jpfhir.jp/fhir/core/mhlw/CodeSystem/ICD10-2013-full、レセプト電算処理用傷病名コード：http://jpfhir.jp/fhir/core/mhlw/CodeSystem/masterB-disease）

code.texはコード化の有無にかかわらず病名入力文字列を必ずそのまま設定する。なお、修飾語は前置修飾語と後置修飾語にわけて、それぞれの拡張を使用して記述する。
